2026 Summer Flags RELAY SIGNUP

Use this form to sign your swimmers up to have their leg of a relay photographed. The only way I can know which relay your swimmer is on (A, B, C, etc) and which leg is if you tell me. This information is not in the heat sheet or in meet mobile.

This form is separate from the $60 individual events form so you can wait to fill this form out once you have relay information from the coach.

DO NOT LIST INDIVIDUAL EVENTS HERE. ONLY LIST FACTUAL RELAY INFORMATION (NOT SPECULATION).

If your coach changes relays after you submit this you can try texting me as soon as you get that information and I'll try my best to update my heatsheet in time. 478-718-1893.

Swimmer First Name (required)

Type the first name as it appears in the heat sheet even if the heat sheet has the name incorrect otherwise it's harder to find your swimmer.

Swimmer Last Name (required)

Type the last name as it appears in the heat sheet even if the heat sheet has the name incorrect otherwise it's harder to find your swimmer.

Swimmer Age and Gender (required)

If your swimmer is younger than 6, pick 6. If older than 18 pick 18. F for Female and M for Male.

Team Name and Team Code (required)
